IN ORDER TO PARTICIPATE IN ATHLETICS, CHEER, AND MARCHING BAND, ALL PARTICIPANTS (7TH – 12TH GRADE)
 
MUST MEET THE REQUIREMENTS ESTABLISHED BY THE UNIVERSITY INTERSCHOLASTIC LEAGUE (UIL).

RANK ONE INSTRUCTIONS
• To access the online forms, go online to New Deal ISD website and click on Athletics. Scroll down to Sports 
Medicine and click on Rank One. Once you are in Rank One; select “Proceed to Online Forms”. You will see a list of 
Electronic Forms to be submitted online. It is not necessary to create an account. You may sign in as a guest. 
• Click on the form name and fill out the information requested.
• To sign the document, click inside the signature box and hold your mouse over the box. This will allow you to create 
an “Electronic Signature.” If you make a mistake and need to start over, click the refresh icon next to the signature 
box.
DO NOT USE STUDENT NICKNAMES ON FORMS and USE STUDENT LUNCH NUMBER (LAST 4 DIGITS) FOR 
STUDENT ID NUMBER.
 
 
 
 
 
EACH YEAR THE FOLLOWING ELECTRONIC FORMS MUST BE COMPLETED AND SIGNED BY 
BOTH A PARENT/GUARDIAN AND THE STUDENT THROUGH NEW DEAL ISD RANK ONE PAGE.
PLEASE COMPLETE FORMS ASAP for 2026-2027 school year.
 
 
1). PRE-PARTICIPATION PHYSICAL EXAMINATION: Upon entering the first year of Middle School and first and third years of High School, a physical examination is required. Physicals are considered current for two years from obtained physical date. All grade level participants (7th -12th) must have a current physical on file with the school.
 
2). MEDICAL HISTORY FORM: *EACH YEAR* prior to any practice or participation, a UIL medical history form must be completed/signed by the parent/guardian and student and submitted. 
 
3-6). UIL FORMS SIGNATURE PAGE: *EACH YEAR* Acknowledgement of Rules, Concussion Acknowledgement, Sudden Cardiac Arrest, Awareness and Parent/Student Anabolic Steroid Use and Random Steroid Testing (high school only) form(s). Forms must be completed/signed by the parent/guardian and student and submitted. 
 
7-9). NDISD MEDICAL FORMS: *EACH YEAR* Emergency Contact Information, Health Information Policy, Over the Counter (OTC) Medication Parent Permission Form(s). Forms must be completed/signed by the parent/guardian and student and submitted.

REQUIREMENTS TO RECEIVE A PHYSICAL AT THE SCHOOL:

  1. *Completion of UMC Physicians Patient Registration Form (black/blue ink only). MUST BE TURNED IN TO MS/HS OFFICE BY THURSDAY, APRIL 30TH 2026!!! UMC will need to input all information in their system prior to Physicals on May 7th.
  2. *Completion Of Medical History Form / top line of Physical Form (black/blue ink only). Answer questions and explain “YES” answers in appropriate box. Parent/student signature required!
  3. Student must bring completed Medical History Form with payment (cash or Medicaid Member #).

Your child will not receive a Physical if the Medical History is not completed and signed by the parent/student. Any “YES” answers to questions 1, 2, 3, 4, 5, or 6 requires further medical evaluation/written clearance for you child’s PCP / Specialist.  In this case, we would recommend using your PCP instead of receiving a Physical through UMC Idalou Clinic (at school).
